To facilitate the service process of equipment purchased in our company, we have prepared a special application form. Please complete carefully so that we have all the necessary information needed to react quickly to any service faults.

A short instruction in case you need to send your device:

1. The device should be packed preferably in the original box (if the original box is missing, the device should be well secured so that it will not be damaged during transport).
2. The device should be accompanied by a technical passport of the device (if no passport is available, a copy of the purchase invoice).
3. Please attach a note with a description of the defect, contact to the requester, invoicing details and return shipping address.
